Fructose has a glycemic index of 19. Table sugar is 65. Glucose itself is 100. On paper, fructose looks like one of the safer sweeteners on the shelf.
That number is measuring the wrong thing.
Glycemic Index Measures Blood Sugar — Not Liver Load
Glycemic index tracks how much a food raises blood glucose. Fructose barely moves blood sugar at all, which is exactly why it scores so low. But that's because fructose mostly skips blood sugar regulation entirely and goes straight to one organ: the liver.
Glucose is processed by cells throughout the entire body — muscles, brain, organs, all of it. Fructose is different. Nearly all of it is metabolized in the liver, and only the liver.
What the Liver Does With It
When the liver receives fructose in small, occasional amounts — a piece of fruit, for example — it's not a problem. The liver converts it to energy or stores a small amount as glycogen, the same way it handles other carbohydrates.
The issue is volume. Fructose today doesn't mostly arrive from fruit. It arrives from high fructose corn syrup, agave, fruit juice concentrate, and "natural" sweeteners stacked into sports drinks, sauces, and packaged snacks — all day, in amounts a liver didn't evolve to process that often.
When fructose shows up faster and more often than the liver can use it for energy, the liver converts the excess into fat through a process called de novo lipogenesis. Consumed occasionally, this is manageable. Consumed constantly — which is how most people encounter fructose now, often without realizing it — that repeated fat production is what's linked to fat accumulating in liver tissue over time.
A food's glycemic index only tells you what it does to your blood sugar in the few hours after you eat it. It says nothing about which organ is doing the metabolic work, or how often that organ is being asked to do it. That's the gap between "low GI" and "liver-friendly."

Fructose showing up on a label rarely says "fructose." It's usually wearing one of these names instead:
- Sports and "performance" drinks — sucrose and fructose blends for fast absorption
- Salad dressings and pasta sauces — high fructose corn syrup in savory foods most people never think to check
- "No added sugar" fruit products — fruit juice concentrate is still concentrated fructose
- Agave nectar — marketed as a natural alternative, up to 90% fructose
- Coffee creamers — corn syrup solids and fructose-derived sweeteners hiding behind "natural flavor," a catch-all label term
What to Actually Check On a Label
Skip the GI number as your first filter. Instead, scan the ingredient list for: high fructose corn syrup, fructose, fruit juice concentrate, agave nectar or syrup, and "invert" sugars — all of which route most of their load straight to the liver, regardless of what the front of the package claims or how low the sugar grams look.
A low glycemic index tells you fructose won't spike your blood sugar. It tells you nothing about the liver, which is where nearly all of it actually goes. The two are different questions — and only one of them shows up on the label as a number you're taught to check.
